Eyelid surgery (blepharoplasty) includes operations for eyelid malpositions such as entropion, ectropion, dermatochalasis, ptosis and eyelid retraction. The eyelid position can be corrected and blepharoplasties performed. Eyelid surgery is also performed for different types of skin cancer. The cancer is removed and the eyelid is then reconstructed. Types of Eyelid Malposition

Eyelid malpositions make your eyelids appear tired, droopy or aged. It is a condition that can be congenital, meaning you were born with it, or it could be acquired later in your life. Ptosis can happen in one or both of your eyes and may occur due to age, trauma or various medical disorders. If your condition obstructs your vision, you will require medical intervention. Here are the 5 main types of eyelid malposition:

  1. Ptosis: As explained in the paragraph above this refers to a low position of the upper eyelid and it can be ascribed to a number of causes
  2. Dermatochalasis – This is excess skin, muscle or fat or a combination of any of these three problems in either the upper or the lower eyelids. This results in a tired look or “baggy eyes”. Dermatochalasis may also obstruct the vision or cause irritation to the eyes.
  3. Ectropion – a condition where the upper or lower eyelid and eyelashes turn outward. This can be caused by gradual stretching of the eyelid tissue during life. An ectropion can also be caused by facial cancer, trauma or from previous eye surgery. The tears can no longer be drained and it may cause excessive dryness or tearing as well as resulting in corneal ulcers. This can threaten your vision.
  4. Entropion –This is a condition where the upper or lower eyelid turns inwards. The eyelashes rub against the eye which causes scratching and tearing. Scarring of the cornea and crusting due to a mucus discharge can occur.
  5. Eyelid retraction – This retraction causes irritation and most commonly results from scarring of the little muscles of the eyelid and makes the eye appear more open. 

Eyelid surgery for lid malpositions

Patients with ptosis require lifting of the eyelids. Dermatochalasis patients require blepharoplasties. Entropion and ectropions require repositioning of the eyelids with possible skin grafts and reattachment of the retractors

Eyelid reconstructions for skin cancer are typically performed by an Ophthalmologist. Procedures include incisional biopsies (where a diagnosis is confirmed), an excisional biopsy with frozen sections and reconstructive surgery. In rare cases, having the eye removed is sometimes medically necessary.
